About Ευγονία - Μονάδα Εξωσωματικής Γονιμοποίησης

Η χειρουργική λήψη σπέρματος προτείνεται σε περίπτωση αζωοσπερμίας, αδυναμίας εκσπερμάτισης ή σε αποτυχία προκλητής εκσπερμάτισης με ηλεκτροδιέγερση. Η επέμβαση γίνεται την ημέρα της ωοληψίας με ενδοφλέβια ή τοπική αναλγησία. Ο ορχικός ιστός που λαμβάνεται υφίσταται ειδική επεξεργασία από τον Εμβρυολόγο με σκοπό την ανεύρεση σπερματοζωαρίων. Μετά την απομόνωση κινητών σπερματοζωαρίων ακολουθεί γονιμοποίηση των ωαρίων με τη μέθοδο της μικρογονιμοποίησης. Η περίσσεια των σπερματοζωαρίων είναι δυνατό να καταψυχθεί για μελλοντική χρήση. Υπάρχουν διάφορες μέθοδοι χειρουργικής λήψης σπερματοζωαρίων, είτε από την επιδιδυμίδα είτε από τους όρχεις.

Λήψη σπερματοζωαρίων από την επιδιδυμίδα

Μικροχειρουργική αναρρόφηση σπερματοζωαρίων (microsurgical epididymal sperm aspiration – MESA): Συνιστάται σε περιπτώσεις αποφρακτικής αζωοσπερμίας με φυσιολογική σπερματογένεση. Περιλαμβάνει χειρουργική διάνοιξη της επιδιδυμίδας με χρήση μικροσκοπίου και αναρρόφηση του επιδιδυμικού υγρού για ανεύρεση σπερματοζωαρίων.
Διαδερμική αναρρόφηση σπερματοζωαρίων (percutaneous epididymal sperm aspiration – PESA): Συνιστάται σε περιπτώσεις αποφρακτικής αζωοσπερμίας με φυσιολογική σπερματογένεση. Πραγματοποιείται με παρακέντηση της επιδιδυμίδας με χρήση βελόνας 19-21G.

Λήψη σπερματοζωαρίων από τον όρχι

Αναρρόφηση διά λεπτής βελόνης (fine needle aspiration – FNA): Συνιστάται σε περιπτώσεις αποφρακτικής αζωοσπερμίας με φυσιολογική σπερματογένεση. Πραγματοποιείται με παρακέντηση του όρχεως με χρήση λεπτής βελόνας 21G.
Αναρρόφηση σπερματοζωαρίων από τον όρχι (testicular sperm aspiration-TESA): Συνιστάται σε περιπτώσεις αποφρακτικής αζωοσπερμίας με φυσιολογική σπερματογένεση.
Βιοψία όρχεως (testicular sperm extraction – TESE): Συνιστάται σε περιπτώσεις αποφρακτικής και μη αποφρακτικής αζωοσπερμίας και περιλαμβάνει ανοικτή βιοψία και συλλογή μικρών τεμαχιδίων ορχικού ιστού.

Οι μέθοδοι αναρρόφησης σπερματοζωαρίων από την επιδιδυμίδα (MESA, PESA) ή τους όρχεις (TESA) είναι απλούστερες, όμως είναι σε μεγάλο βαθμό «τυφλές», προκαλώντας βλάβες στον ορχικό ιστό, όπως αιμάτωμα, ίνωση και ορχική ατροφία. Η βιοψία όρχεως (TESE) προτείνεται από τη διεθνή βιβλιογραφία ως η περισσότερο αποτελεσματική μέθοδος ανεύρεσης σπερματοζωαρίων και σχετίζεται με μικρότερο ποσοστό επιπλοκών. Για το λόγο αυτό, στη Μονάδα μας, η χειρουργική λήψη σπερματοζωαρίων γίνεται με βιοψία όρχεως (TESE) από εξειδικευμένους χειρουργούς.Η επέμβαση γίνεται την ημέρα της ωοληψίας με ενδοφλέβια ή τοπική αναλγησία. Τα τεμαχίδια όρχεως υφίστανται ειδική επεξεργασία από τον εμβρυολόγο με σκοπό την ανεύρεση σπερματοζωαρίων. Μετά την ανεύρεση σπερματοζωαρίων ακολουθεί μικρογονιμοποίηση. Η περίσσεια σπερματοζωαρίων είναι δυνατόν να καταψυχθεί για μελλοντική χρήση, χωρίς να απαιτηθεί νέα βιοψία όρχεως.

Διαγνωστική βιοψία όρχεως

Η διαγνωστική βιοψία όρχεως πριν την ημέρα της ωοληψίας δεν συνιστάται. Η θέση αυτή αντικατοπτρίζει την πρακτική της Μονάδας «Ευγονία», σύμφωνα και με τη βιβλιογραφία, διότι: αποτελεί μία επί πλέον χειρουργική επέμβαση, προκαλεί έναν επί πλέον τραυματισμό του όρχεως και κυρίως η ανεύρεση σπερματοζωαρίων στη διαγνωστική βιοψία δεν εγγυάται την ανεύρεση σπερματοζωαρίων την ημέρα της ωοληψίας. Η διαγνωστική βιοψία και η συνακόλουθη ιστολογική εκτίμηση αφορά κυρίως τη σταδιοποίηση των διαφόρων μορφών σπερματογένεσης. Η ιστολογική εξέταση μπορεί όμως να γίνει και από τα τεμαχίδια που λαμβάνονται από τον όρχι την ημέρα της ωοληψίας.

From my personal experience one thing that made ‘the journey’ easier was our medical team at ‘Eugonia ivf Unit’! We trusted the team right from the beginning. Not only because they are attentive, happy to explain everything and accessible. We were pleased, we found a team with such medical expertise, knowledge and focus on what they are doing, practicing according to international standards and following guidelines. Although the unit and staff are very busy, we never felt rushed. At any given stage doctors and nursing staff did what they were supposed to do and where possible we were part of the decision making process. Although it wasn’t easy to conceive, I don’t remember a thing of the stress and the pain now! Our baby girl arrived on the 10th July 2018. It is a little miracle! I often say this baby is made of a lot of love, care and science! Our medical team became our family and we will forever thank them for that!

I would like to share my experience with those Ladies out there who are in the lookout for solutions to end up their fertility struggles. I am a 40 years old lady, UK resident, diagnosed with very low ovarian reserve and this year I have received the most terrifying news that I am at my end of my reproductive life, being given by an NHS UK Fertility Clinic an horrific prognostic for a pregnancy outcome through IVF of 1%. Beating the odds and statistics Dr. George Lainas and his Team has made it possible. I am such a very happy lady today! It is a happiness the words cannot describe… I am speechless… Miracles DO happen after all in the hands of FANTASTIC specialists. I could see the heartbeat of my little angel today. There are so many things about the way Eugonia is set up that made the whole process very positive overall including the daily appointment, regular face to face meeting with the consultant, practising the embryo transfer/ mock embryo- transfer process prior to treatment (which I came to find out later that not all IVF clinics are offering it, yet it may be the thing that could sometimes make the difference in the IVF outcome), the relaxed environment and the calm and measured staff. Cumulatively, these features reinforced the feeling of being treated as individual rather than being lumped onto the conveyor belt. Eugonia has made me feel at home, away from home for almost 2 weeks and a half, while the proceedings of the IVF took place. Experience, knowledge, caring, compassion, understanding, empathy, and integrity are surely some values that Eugonia lives by. E	Eleftheria Alexiou on Google ★ ★ ★ ★ ★ We scheduled a treatment at your center for the end of July 2021 having decided to proceed to the Natural Cycle - IVF. We had informed you about this decision multiple times verbally and via emails. Our wish as well as other doctors' advice NOT to receive a normal IVF protocol but a NATURAL CYCLE IVF, were both totally disrespected and ignored. The doctor decided for me to follow the protocol "Antagonists" against my wish and decision. I heard by incident from a nurse that I do not follow a Natural Cycle protocol but the "Antagonists!!! I asked to see the doctor immediately after I heard about this, and after a few days of absence Dr. Georgios Lainas appeared, and told me "you have to feel lucky we made a mistake, because now you have 5 eggs instead of 1-2". Furthermore, he said "as it was our mistake, so we decided that you will pay the final amount of 1'500€ instead of 2'800€". Something very untrue as we finally paid approximately 3'000€ to the Eugonia fertility center with all the additional "hidden" costs that would cover the loss of the financial mistake!!! My husband and I asked multiple times to close the bill during the procedure and the receptionists always replied that it was not the time. And finally, I was called to pay the bill exactly after the egg retrieval feeling dizzy from the anesthesia having given a piece of paper with a sum similar to those in the fruit markets (see attached) written by the "financial manager" of the clinic. The same person was chasing me in the clinic exactly after the embryo transfer forcing me to sign two promissory loan checks that referred to the potential eggs for freezing, even before the embryo reassuring report for the adequacy of the embryos. The embryologists of your center misled us to make very bad decisions. After visiting one of the most well known fertility clinics and doctors in Switzerland, they informed us that the described embryos were extremely poor for transfer and even more inappropriate for freezing (category 4CC also described in a written report). They informed us in detail that the freezing procedure on the 5th and 6th day is unacceptable as well as the progress of the embryos was very bad and against all the international protocols and standards. They also assured us that the unsuccessful result of the transfer of the described embryos as well as a potential use of the frozen embryos were both inappropriate and the result definitely unsuccessful. Totally unhappy with the medical mistakes as well as all the poor quality of services.
